Warfarin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the short version with warfarin and grapefruit: grapefruit can slow down one of the liver enzymes (CYP3A4) that helps clear warfarin from your body. When that enzyme is partly blocked, warfarin can hang around longer and build up. That means your blood may get thinned more than intended, which raises the chance of bleeding problems like easy bruising, nosebleeds, bleeding gums, blood in the urine or stool, or worse.
Warfarin is a narrow therapeutic index drug, so even a small shift can push your INR out of range. The good news is this is manageable. The issue is usually changing your grapefruit habits suddenly, not a steady, predictable amount. Don't start or stop grapefruit on your own. Talk with your pharmacist or doctor so your INR can be watched.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

If that pathway slows down, warfarin can hang around a bit longer, and its blood-thinning effect could creep up. Warfarin is a narrow-therapeutic-index drug, meaning even small shifts can matter, so a stronger effect could show up as easy bruising, nosebleeds, bleeding gums, or a higher INR on your bloodwork.
That said, this is mostly theoretical and based on lab and limited data, not strong warfarin studies. An occasional grapefruit probably isn't a crisis, but regular grapefruit or juice is worth running past your pharmacist or doctor, especially since they can check your INR.

WARFARIN (Coumadin)
5 of 5In one case report, a patient experienced significantly increased international normalized ratio (INR) associated with consumption of 50 ounces of grapefruit juice daily. However, smaller amounts of grapefruit juice might not be a problem. In a small clinical trial, consumption of 24 ounces of grapefruit juice daily for one week had no effect on INR in males treated with warfarin.

Here's the honest picture with grapefruit and warfarin. Grapefruit can slow down some of the liver enzymes that help clear warfarin out of your body. If those enzymes slow down, warfarin can build up a little and thin your blood more than intended. Since warfarin is a narrow therapeutic index drug, even small shifts can nudge your INR up and raise the risk of bruising or bleeding (think nosebleeds, bleeding gums, or blood in the urine or stool).
That said, the evidence here is weak. It comes mainly from a single case of someone drinking about 50 ounces of grapefruit juice a day. A small study using 24 ounces daily showed no change in INR. So an occasional grapefruit is unlikely to matter, but large daily amounts could. Please check with your pharmacist or doctor before making it a habit, and don't change anything on your own.

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
