OXYCODONE (Oxycontin)

Grapefruit juice can increase blood levels of oxycodone, potentially increasing the effects and adverse effects of oxycodone.
Oxycodone is metabolized by both cytochrome P450 3A4 (CYP3A4) and cytochrome P450 2D6 (CYP2D6). A small clinical study shows that grapefruit juice can increase plasma levels of oral oxycodone about 1.7-fold by inhibiting CYP3A4. While the analgesic effects of oxycodone do not seem to be affected, taking grapefruit juice along with oxycodone may theoretically increase the adverse effects of oxycodone.

Here's the short version: grapefruit can slow down one of the liver enzymes (CYP3A4) that helps clear oxycodone from your body. When that enzyme is blocked, more oxycodone hangs around in your bloodstream. In one small study, grapefruit juice raised oxycodone levels by about 1.7 times.

Oxycodone is an active painkiller (not a prodrug that needs switching on), so higher levels mean a stronger opioid effect. Interestingly, the pain relief didn't seem to change much, but the side effects may climb, things like extra drowsiness, dizziness, constipation, nausea, and in larger amounts, slowed breathing. An occasional bite of grapefruit is unlikely to be the issue; regular glasses of juice are the bigger concern. If you enjoy grapefruit often, just run it by your pharmacist or doctor first.
