Why this applies to Irbesartan
Irbesartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Grapefruit interacts with.
In vitro research suggests that grapefruit juice might inhibit CYP2C9 enzymes. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with grapefruit and irbesartan. Your body partly clears irbesartan using a liver enzyme called CYP2C9, and lab (test-tube) studies hint that grapefruit might slow that enzyme down. If that happened in real people, a little more irbesartan could linger in your system, which could nudge its blood-pressure-lowering a bit too far. That might show up as dizziness, lightheadedness when you stand up, or feeling unusually tired.
The important part: this is only a theoretical concern from laboratory work, and it has not actually been reported in humans. So I wouldn't panic over an occasional grapefruit or a glass of juice. Still, if you eat or drink grapefruit regularly, mention it to your pharmacist or doctor so they can keep an eye on your readings.

What Is Irbesartan Used For?

Irbesartan lowers high blood pressure (hypertension). Lowering blood pressure reduces the risk of strokes and heart attacks. It may be used alone or with other blood pressure medicines.
It is also used to treat diabetic nephropathy (kidney disease from diabetes). This applies to people with type 2 diabetes, high blood pressure, raised serum creatinine and protein in the urine. In this group, it slows how fast the kidney disease gets worse.
