Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the simple version: irbesartan (Avapro) is a blood pressure medicine, and adrafinil turns into modafinil in your body, which is a stimulant. Stimulants can nudge your blood pressure up a little. So the worry is that adrafinil might work against your irbesartan, making it less effective at keeping your numbers down.

To be fair, this is a theoretical concern based on how these drugs behave, not strong proof. In studies, the effect on blood pressure was usually small. Still, if you're taking irbesartan to protect your heart, brain, and kidneys, you don't want it quietly losing ground. If you use adrafinil, check your blood pressure at home and let your pharmacist or doctor know, so they can keep an eye on things.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the short version: your body clears irbesartan partly through a liver enzyme called CYP2C9. Adrafinil turns into modafinil in your liver, and lab studies suggest modafinil may dial down that same enzyme. If that happens, irbesartan could clear a little slower and build up, which might make its blood-pressure-lowering effect stronger than expected.

In real life that could mean more dizziness, lightheadedness (especially when standing up), or feeling unusually tired. I want to be honest though: this is a theoretical concern based on test-tube data, not proven in people, so it may not amount to much. Still, since adrafinil is unregulated and hard on the liver, please run it by your doctor or pharmacist before combining the two.

What Is Irbesartan Used For?

Brand names: Avapro, Aprovel

Irbesartan tablets — sold as the brand Avapro and as generic irbesartan — are used to treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of serious events like stroke and heart attack. It can be used alone or alongside other blood pressure medicines.

Irbesartan tablets (including Avapro) are also used to treat diabetic nephropathy — kidney disease caused by diabetes — in people who have type 2 diabetes, high blood pressure, and signs of kidney damage. In this group, it helps slow how quickly the kidney disease gets worse.
