Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with these two together. Adrafinil turns into modafinil in your body, and modafinil can mildly block a liver enzyme called CYP2C19. The idea on file is that this slowdown could let drugs cleared by that enzyme build up a bit. Formoterol is not a prodrug, so if it were affected, you'd expect slightly stronger effects.
The good news: this is a theoretical concern based on lab data, not real-world reports, and formoterol is breathed into your lungs and mostly handled by other pathways, so a big interaction is unlikely. Still, if you ever notice a faster heartbeat, jitteriness, tremor, or trouble sleeping, mention it to your pharmacist or doctor before changing anything.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Adrafinil and your Formoterol inhaler. Your body turns adrafinil into modafinil, and in lab studies modafinil may slow down a liver enzyme called CYP2C9. Formoterol is partly handled by that enzyme, so in theory it could build up a bit and push its effects harder than usual.
If that happened, you might notice more of formoterol's typical side effects, like a fast or pounding heartbeat, jitteriness, shakiness, or trouble sleeping. I want to be honest though: this is only a theoretical concern based on test-tube data, not real patient reports, so the actual risk is likely small. Since adrafinil is unregulated and tough on the liver, please run it by your doctor or pharmacist before combining the two.

What Is Formoterol Oral Inhalation Used For?
Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler
Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.
Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
