Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.

Here's the basic idea: when your body turns adrafinil into modafinil, that modafinil may gently speed up a liver enzyme called CYP1A2. Since acetaminophen (Tylenol) is partly handled by that same enzyme, the theory is that your body could clear it a little faster, which might make a dose feel slightly less effective for pain or fever.

I want to be honest about how shaky this one is. It's only theoretical, based on lab tests showing modafinil is a weak enzyme booster, and acetaminophen isn't a narrow-margin drug. So a real-world change in how well your Tylenol works is unlikely. Still, since adrafinil is unregulated and hard on the liver (and so is too much acetaminophen), please run this combo past your pharmacist or doctor first.

What Is Acetaminophen Used For?

Acetaminophen temporarily relieves minor aches and pains caused by headaches, the common cold, backaches, muscle aches, toothaches, minor arthritis pain, and premenstrual or menstrual cramps. It also temporarily reduces fever.

These uses apply across oral tablet, suspension, and oral liquid products. The intravenous (IV) injection form is used in clinical settings to treat acute pain and fever, including in pediatric patients 2 years and older.
