Here's the gist for this pair. Cyclosporine can mildly block an enzyme in your liver called CYP3A4. That same enzyme helps clear modafinil, which is what your body turns adrafinil into. So in theory, taking cyclosporine alongside adrafinil could let modafinil build up a little, meaning stronger or longer-lasting stimulant effects like trouble sleeping, jitteriness, headache, a racing heart, or anxiety.

I'll be honest with you though: this concern is theoretical, based on how these drugs work on paper, not on real cases. There's also a separate worry, adrafinil can stress the liver, and cyclosporine is no friend to your kidneys, so adding an unregulated supplement isn't something I'd do without a green light. Please check with your doctor or pharmacist first.

Here's the gist for cyclosporine. Your liver clears this medication using an enzyme called CYP3A4. Once your body turns adrafinil into modafinil, that modafinil can weakly speed up that same enzyme. In theory, that means your liver could clear cyclosporine a little faster, nudging its blood levels down.

Why that matters: cyclosporine is a narrow therapeutic index drug, so even small dips can be a problem. If you're a transplant patient, lower levels raise the worry of organ rejection. For rheumatoid arthritis or psoriasis, you might just see less benefit. Keep in mind this is a theoretical, lab-based concern, not a proven one. Still, please loop in your transplant team or pharmacist before mixing these, since they can check your levels.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
