Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to keep in mind if you take diltiazem (Cardizem and similar) for blood pressure or chest pain. Adrafinil turns into modafinil in your body, and modafinil is a stimulant that tends to nudge blood pressure and heart rate upward. Since diltiazem works to relax your blood vessels and ease the heart's workload, adding a stimulant could work against it and partly cancel out the benefit you're trying to get.

In real terms, you might notice your blood pressure creeping back up, more chest tightness, or a faster heartbeat. I'll be honest, this is a theoretical concern based on how these drugs behave, not strong proof, so don't panic. Just loop in your doctor or pharmacist before combining them, and keep an eye on your readings if you do.

Here's the gist for this pairing. Adrafinil is a prodrug that your liver turns into modafinil, and modafinil is cleared from your body by a liver enzyme called CYP3A4. Diltiazem happens to slow down that same enzyme. So in theory, taking diltiazem could let modafinil hang around longer and build up, making the stimulant effects feel stronger. That might mean more jitteriness, trouble sleeping, a racing or pounding heart, headache, or higher blood pressure.

This is a theoretical concern based on how the drugs work, not something proven in studies, so don't panic. Still, since adrafinil is unregulated and hard on the liver, please check with your doctor or pharmacist before combining it with diltiazem.

Here's the gist for these two together. Once your liver turns adrafinil into modafinil, modafinil can mildly speed up an enzyme called CYP3A4, which is one of the main enzymes that breaks down diltiazem. If that enzyme works a little faster, your body may clear diltiazem more quickly, leaving lower levels in your blood. In plain terms, your diltiazem might not work quite as well, so your blood pressure could creep up or your chest pain (angina) could become harder to control.

I'll be honest, this is a theoretical concern based on how the drugs behave, not strong real-world data, so it may not amount to much for many people. Still, before combining them, please check in with your pharmacist or doctor so they can keep an eye on your blood pressure and symptoms.

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
