Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the gist of pairing these two. Adrafinil turns into modafinil in your body, and modafinil tends to nudge blood pressure upward. Since nifedipine's whole job is to relax your blood vessels and bring blood pressure down, the two can work against each other. In plain terms, the supplement may chip away at how well your nifedipine controls your numbers.

If that happens, you might notice your blood pressure creeping back up, or chest pain (angina) not being as well controlled as before. To be clear, this is a theoretical concern based on how modafinil behaves, not something proven specifically with nifedipine, so don't panic. Still, since this is unregulated and hard on the liver, please check with your doctor or pharmacist before combining them, and keep an eye on your blood pressure readings.

Here's how this pairing works. Your blood pressure medicine, nifedipine, gently blocks a liver enzyme called CYP3A4. Adrafinil leans on that same enzyme. After your body turns adrafinil into modafinil (the part that actually keeps you awake), CYP3A4 is one route used to clear it out. If nifedipine slows that enzyme down, modafinil may hang around a bit longer, which could mean more stimulant-type effects like jitteriness, a racing heart, trouble sleeping, headache, or higher blood pressure.

I want to be honest: this is a theoretical concern based on how the drugs work, not strong patient studies, so it may not cause real trouble. Still, adrafinil is unregulated and hard on the liver. Please check with your doctor or pharmacist before combining them.

Here's the gist for nifedipine. Your liver uses an enzyme called CYP3A4 to break this medicine down. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could clear nifedipine a little faster and weaken it, so your blood pressure or chest pain (angina) might not be as well controlled.

I want to keep this in perspective. This concern is theoretical, based on how the drugs work rather than real cases, and the effect is thought to be weak. What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
