Here's the gist for phenytoin (Dilantin). Adrafinil turns into modafinil in your body, and modafinil can slow down a liver enzyme called CYP2C19, one of the pathways that helps clear phenytoin. If that pathway is blocked, phenytoin can build up in your blood. Since phenytoin is a narrow therapeutic index drug, even small increases can tip you into side effects like dizziness, drowsiness, slurred speech, unsteady walking, or jerky eye movements.

I'll be honest though: this concern is theoretical, based mostly on lab data, not real patient reports. Still, with phenytoin I'd play it safe. Please check with your doctor or pharmacist before combining these, since they may want to monitor your phenytoin levels.

Here's the gist: your body changes adrafinil into modafinil, and lab studies hint that modafinil may slow down an enzyme called CYP2C9. That same enzyme is one of the main ways your liver clears phenytoin. If that pathway slows, phenytoin can build up in your blood, and that matters a lot because phenytoin has a very narrow safe range. Even a small bump can tip you over.

Higher phenytoin levels can show up as dizziness, slurred speech, unsteady walking, double vision, drowsiness, or nausea. I'll be honest though, this interaction is theoretical and comes from test-tube data, not real patients, so it may not happen at all. Still, with a drug like phenytoin I'd play it safe and check with your doctor or pharmacist before combining the two.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
