Here's the thing to know about pairing these two: the concern runs from prednisone toward adrafinil, not the other way around. Adrafinil is a prodrug that your liver turns into modafinil, and that modafinil is cleared by a liver enzyme called CYP3A4. Prednisone can mildly nudge this same enzyme, so in theory it could shift how much active modafinil sticks around. If levels climb, you might notice more of modafinil's effects, like trouble sleeping, jitteriness, headache, or a faster heartbeat.

I'll be straight with you though: this is a theoretical interaction based on how the drugs work on paper, not on real patient reports, so I wouldn't lose sleep over it. Since adrafinil is unregulated and hard on the liver, please run this combo by your pharmacist or doctor first.

Here's the gist for these two together. Your liver turns adrafinil into modafinil, and modafinil can mildly speed up an enzyme called CYP3A4. That same enzyme helps break down prednisone. So in theory, taking adrafinil could clear prednisone out a little faster, which might make your steroid dose work a bit less than expected.

If prednisone is keeping a flare of arthritis, asthma, allergies, or another condition under control, weaker effect could mean those symptoms creep back. Keep in mind this is a theoretical concern based on how the drugs behave, not strong proof, and the effect on CYP3A4 is considered weak. Still, since adrafinil is unregulated and hard on the liver, please run it by your doctor or pharmacist before combining them.
