STIMULANT DRUGS
Why this applies to Phentermine
Phentermine falls under STIMULANT DRUGS — the group Adrafinil interacts with.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: phentermine is already a stimulant that revs up your system to curb appetite. Adrafinil turns into modafinil in your liver, and modafinil is also a wakefulness booster. So if you stack the two, you're basically piling one stimulant on top of another. The concern is that their effects could add up.
In real life, that might look like a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, headache, or feeling wired and anxious. I'll be honest, this warning is theoretical, based on how these drugs work rather than solid studies, so I don't want to scare you. Still, both are stimulants, so please check with your doctor or pharmacist before combining them.

This page covers Adrafinil only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Phentermine page lists 61 supplements it has a documented interaction with, worst first, each one explained.
The most serious there right now: Activated Charcoal (major), Ephedra (major), Peyote (major), Rauvolfia Vomitoria (major) and Sida Cordifolia (major).
By severity: 5 major · 43 moderate · 13 minor.
From our pharmacist’s overview on the Phentermine page
“Ephedra is the pairing I most want anyone taking phentermine (Adipex-P) to know about. Phentermine is itself a stimulant appetite suppressant that raises heart rate and blood pressure, and ephedra piles the same cardiovascular strain on top of it.”
