Everolimus falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's how this one works, and the direction matters. Everolimus is a mild blocker of the liver enzyme CYP3A4, the same enzyme that turns adrafinil into its active form, modafinil. So if you took both, everolimus could theoretically slow that conversion a little, leaving you with somewhat less of the wakefulness effect you were hoping for from the adrafinil.

I want to be honest about how soft this is: it's based on theory and pharmacology, not real human studies, so it may not amount to much in practice. The bigger concern is adrafinil itself, which is unregulated and can be hard on the liver, something you don't want while on cancer therapy. Please loop in your oncologist or pharmacist before combining them.

Here's the situation with these two. Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Everolimus is cleared from your body by that exact enzyme. So if it runs a little faster, your everolimus levels could drop, and that may mean the drug works less well against the cancer or tumors it's treating.

I want to be honest about the strength of this: it's theoretical, based on how the drugs behave, not on real patient cases. But everolimus is a serious medication where blood levels really matter, so even a small shift is worth taking seriously. Please don't change anything on your own. Loop in your oncologist or pharmacist before mixing these.

About Everolimus

What Is Everolimus Used For?

Brand names: Certican, Afinitor, Zortress, Afinitor Disperz

Everolimus is used to treat several serious conditions. In cancer, it is prescribed for advanced hormone receptor-positive, HER2-negative breast cancer (in postmenopausal women), certain neuroendocrine tumors of the pancreas, GI tract, or lung, and advanced renal cell carcinoma (kidney cancer) when other treatments have stopped working.

It is also used for people with tuberous sclerosis complex (TSC) — a genetic condition that causes benign tumors to grow throughout the body. In TSC, it treats kidney tumors called angiomyolipomas, brain tumors called SEGA, and seizures. In kidney transplant patients, it helps prevent the body from rejecting the new organ.
