Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the gist for these two together. Your body turns adrafinil into modafinil, a stimulant that can nudge your blood pressure and heart rate upward. Verapamil is doing the opposite job, relaxing your vessels to lower blood pressure and calm your heart rate. So adrafinil could work against verapamil and make it a bit less effective at keeping your numbers down or your chest pain controlled.

I want to be honest that this is a theoretical concern based on how these drugs behave, not strong proof, and any effect is likely small. Still, if you're using verapamil for blood pressure, angina, or rhythm control, please check with your pharmacist or doctor first and keep an eye on your blood pressure rather than starting adrafinil on your own.

Here's the gist: once your body turns adrafinil into modafinil, that modafinil may gently nudge a liver enzyme called CYP1A2 to work a little faster. Verapamil is partly handled by that enzyme, so in theory it could be cleared a bit quicker, which might make it slightly less effective at controlling your blood pressure or heart rate.

I want to be honest about how soft this is. It's a theoretical concern based only on lab data, and modafinil's effect on this enzyme looks weak. Here's the gist of pairing these two. Verapamil slows down a liver enzyme called CYP3A4, and that same enzyme is the one that clears modafinil, the active drug your body makes after taking adrafinil. So when verapamil is on board, modafinil may linger longer and pile up a bit. In real terms, that could mean stronger stimulant effects like trouble sleeping, jitteriness, headache, a racing or pounding heart, and higher blood pressure, which is a bit counterproductive if you're taking verapamil to keep blood pressure and heart rate in check.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
