Why this applies to Carisoprodol

Carisoprodol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the deal with Soma (carisoprodol) and grapefruit. Your body uses a liver enzyme called CYP2C19 to break carisoprodol down. Grapefruit can slow that enzyme, so the drug may clear out a bit more slowly and build up in your system. Unlike a prodrug such as Plavix (which grapefruit actually weakens), carisoprodol is already active, so higher levels mean a stronger effect.

In plain terms, you might feel more drowsy, dizzy, or foggy than usual. I'll be honest though, this is mostly based on lab and theoretical work, so it's not a sure thing. An occasional grapefruit probably isn't a big deal, but if you eat or drink it regularly, just check with your pharmacist or doctor first.
