Why this applies to Brivaracetam

Brivaracetam falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the gist for Briviact (brivaracetam): your liver uses an enzyme called CYP2C19 to help break this medicine down. Grapefruit can partly block that enzyme, so in theory your body might clear brivaracetam a little slower and let levels creep up. Brivaracetam is an active drug, not a prodrug, so higher levels generally mean stronger effects, including more drowsiness, dizziness, fatigue, or unsteadiness.

I want to be honest about the strength of this, though. It's mostly theoretical and based on lab work, and the real human study was with a different drug (clopidogrel). An occasional grapefruit probably isn't a big deal, but if you drink grapefruit juice regularly, mention it to your pharmacist or doctor so they can keep an eye on how you feel.

What Is Brivaracetam Used For?

Brand name: Briviact

Brivaracetam — available as tablets, oral solution, and injection under the brand name Briviact, as well as generic forms — is indicated for the treatment of partial-onset seizures (seizures that begin in one part of the brain) in patients 1 month of age and older. It can be used alone (monotherapy) or alongside other seizure medicines (adjunctive therapy).

Briviact injection is specifically intended for intravenous use when a patient is temporarily unable to take medicine by mouth — it is not meant for long-term IV use. The oral tablet and oral solution forms are used for ongoing treatment.
