Grapefruit and Flurazepam

Here is the interaction we found between these two, with the evidence behind it.

Major

BENZODIAZEPINES

Why this applies to Flurazepam

Flurazepam falls under BENZODIAZEPINES — the group Grapefruit interacts with.

Grapefruit juice might increase blood levels of some oral benzodiazepines, potentially increasing the effects and adverse effects of these drugs.
Clinical research shows that grapefruit juice can increase plasma triazolam concentrations. Repeated consumption of grapefruit juice greatly increases triazolam concentrations and prolongs the half-life, probably due to inhibition of cytochrome P450 3A4 (CYP3A4). Some studies show that grapefruit juice, particularly when taken in large quantities, reduces the clearance and increases the maximum blood levels, area under the plasma concentration curve (AUC), and duration of effect of midazolam. However, there is no effect on intravenous midazolam. Grapefruit juice has also been shown to increase the maximum blood levels and duration of effect of diazepam, but the clinical significance of this is not known. This interaction does not appear to occur with alprazolam.
Severity Major Likelihood Likely Evidence B

Well-controlled human studies have demonstrated the likely existence of this interaction.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the deal with grapefruit and your sleep medication. Flurazepam is broken down by a liver enzyme called CYP3A4, and grapefruit (especially in larger or repeated amounts) blocks that enzyme. When the breakdown slows, more flurazepam can hang around in your bloodstream than your dose was meant to deliver.

For a sleep aid, that usually means a stronger and longer-lasting effect. You might feel extra drowsy or groggy the next morning, more lightheaded or unsteady on your feet, or just slow to wake up. That matters most for older folks, who can be at higher risk of falls. We've seen this clearly with similar benzodiazepines like triazolam, so it's a reasonable concern here. The occasional bite of grapefruit is one thing, but regular juice or daily fruit is worth a quick chat with your pharmacist or doctor before you combine them.

From our pharmacist’s overview on the Flurazepam page

“Beer and wine sit at the top of this list for good reason. Alcohol taken with flurazepam (Dalmane), a long-acting benzodiazepine sleep medicine, can deepen sedation dangerously, a major-rated and well documented interaction.”

About Flurazepam

What Is Flurazepam Used For?

Brand names: Dalmane, Somnol, Apo-Flurazepam, Bio-Flurazepam, Novo-Flurazepam, PMS-Flurazepam, Som Pam

Flurazepam Hydrochloride capsules are used to treat insomnia. Specifically, they help with difficulty falling asleep, frequent wake-ups during the night, and early morning awakening.

Because insomnia is often short-lived, flurazepam is generally intended for short-term use. If sleep problems don't improve within 7 to 10 days of treatment, a healthcare provider should evaluate whether an underlying medical or psychiatric condition may be the cause.
