Aspirin, Carisoprodol, Codeine Phosphate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

This combination pill mixes aspirin, the muscle relaxer carisoprodol, and codeine. The interaction on file focuses on the CYP2C19 enzyme, which grapefruit may gently block. Carisoprodol leans on CYP2C19 to be broken down, so in theory grapefruit could let it linger a bit longer, nudging up drowsiness, dizziness, or that foggy, sedated feeling.

Honestly, this is mostly lab-based and theoretical, so I wouldn't lose sleep over an occasional grapefruit. Just keep in mind codeine and carisoprodol already cause sleepiness, so adding anything that increases that effect deserves caution. If you enjoy grapefruit regularly, run it by your pharmacist or doctor first, especially before driving or mixing with alcohol.
