Grapefruit and Meloxicam

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Meloxicam

Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that grapefruit juice might inhibit CYP2C9 enzymes. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for meloxicam: your liver relies on an enzyme called CYP2C9 to break it down and clear it. Lab studies hint that grapefruit may slow that same enzyme. If that happened in your body, meloxicam could linger a bit longer and effectively build up, which might nudge up the usual NSAID side effects like stomach upset, heartburn, fluid retention, raised blood pressure, or, with heavier use, kidney strain or an increased risk of stomach irritation.

The honest part: this is only a theoretical concern based on test-tube research, and it hasn't actually been seen in people. An occasional grapefruit is very unlikely to cause trouble. Still, if you eat it often or drink the juice daily, it's worth a quick chat with your pharmacist or doctor to be safe.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Meloxicam page

“Most people take meloxicam (Mobic) daily for arthritis pain, which gives supplement interactions time to add up, so I read this list with real vigilance. Five of the 113 ingredients on file carry a major rating, and two are simply beer and wine, because alcohol plus any NSAID raises the odds of stomach bleeding.”

About Meloxicam

What Is Meloxicam Used For?

Brand names: Mobic, Vivlodex, Qmiiz ODT, Anjeso

Meloxicam tablets and oral suspension treat the signs and symptoms of osteoarthritis, rheumatoid arthritis, and juvenile rheumatoid arthritis (JRA). The oral suspension and Zybic™ oral suspension are approved for JRA in children 2 years and older; meloxicam tablets are approved for JRA in patients who weigh at least 60 kg. Meloxicam capsules are approved specifically for the management of osteoarthritis pain.

Two IV (intravenous) formulations — QAMZOVA and XIFYRM — are indicated for adults for the management of moderate-to-severe pain, used alone or with non-NSAID pain relievers. Because these IV forms take time to reach full effect, they are not recommended when rapid pain relief is needed.
