Fomoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the honest picture for your formoterol (Foradil) inhaler. The interaction on file involves grapefruit possibly slowing down a liver enzyme called CYP2C19, which could let certain medicines build up a bit higher than usual. Formoterol is grouped with drugs touched by that enzyme, so in theory grapefruit might raise its levels slightly.

That said, keep this in perspective. This is mostly theoretical and lab-based evidence, and formoterol is breathed into your lungs rather than swallowed, so very little reaches the liver to begin with. Any real-world effect is likely small. If levels did rise, you might notice more typical formoterol side effects like a fast or pounding heartbeat, jitteriness, or a slight tremor. If you enjoy grapefruit regularly, just mention it to your pharmacist or doctor so they can confirm you're fine.

Here's the gist with formoterol (Foradil) and grapefruit. Lab studies hint that grapefruit can slow down a liver enzyme called CYP2C9, which plays a role in clearing certain medicines. If that enzyme works slower, a drug it handles could build up a bit. For formoterol, more drug in your system could in theory mean more of its typical side effects, like a fast or pounding heartbeat, jitteriness, shakiness, or a headache.

That said, this is purely theoretical. It comes from test-tube research, and it has never actually been reported in people taking this kind of medicine. Most of an inhaled drug works right in the lungs, so the practical risk looks low. Still, if you use formoterol daily and enjoy grapefruit often, it's worth a quick chat with your pharmacist or doctor.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
