Verapamil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the deal with verapamil and grapefruit. Your liver and gut use an enzyme called CYP3A4 to break verapamil down. Grapefruit blocks that enzyme, so less of the drug gets cleared and more of it builds up in your blood. Verapamil is already active as you take it (it's not a prodrug), so higher levels mean a stronger effect than you bargained for.
In real terms, that can mean blood pressure dropping too low, dizziness, lightheadedness, swelling in your ankles, constipation, or a heart rate that slows down too much. This effect can linger a day or two after the grapefruit. Please don't change anything on your own. Clinical research shows that grapefruit juice increases absorption and plasma concentrations of amlodipine, nifedipine, nisoldipine, verapamil, felodipine, nimodipine, nicardipine, diltiazem, pranidipine, nitrendipine, and manidipine, This interaction is likely the result of the inhibition of intestinal metabolism of these drugs by CYP3A4, although some research suggests grapefruit may alter plasma drug levels by reducing the rate of gastric emptying. Consuming grapefruit juice 1 liter daily increases steady state concentrations of verapamil by as much as 50%. However, some references dispute the clinical relevance of the interactions with amlodipine, diltiazem, and verapamil. Other research in healthy individuals suggests plasma levels of felodipine and nifedipine are not affected when given intravenously. There is considerable interindividual variability in the effect of grapefruit juice on drug metabolism, which might account for inconsistent study results. In healthy older adults, the hemodynamic response to felodipine plus grapefruit juice might be influenced by altered autonomic regulation. In older healthy adults, a single dose of grapefruit juice and felodipine enhanced the blood pressure-lowering effects of felodipine. However, after a week of grapefruit juice and felodipine (steady state), the hypotensive activity was reduced, possibly due to compensatory tachycardia. Research indicates it is necessary to withhold grapefruit juice for as long as 3 days to avoid interactions with felodipine and nisoldipine.

Here's the short version: grapefruit can block an enzyme in your gut called CYP3A4, and that's the same enzyme your body uses to break down verapamil. When that enzyme is slowed, more of your verapamil gets absorbed instead of being cleared, so the drug level in your blood can climb. One study found that drinking about a liter of grapefruit juice a day raised verapamil levels by up to 50%.
Since verapamil already lowers blood pressure and slows the heart rate, having more of it on board could push those effects too far. That might show up as dizziness, lightheadedness, unusual tiredness, a very slow pulse, or swelling in the ankles. The research isn't fully consistent, and some experts question how big a deal this is, but because it's your heart, play it safe. In vitro research suggests that grapefruit juice might inhibit CYP1A2 enzymes. So far, this interaction has not been reported in humans.

Here's the short version on grapefruit and verapamil. The interaction we have on file is based only on lab (test-tube) research, where grapefruit appeared to slow down an enzyme called CYP1A2. Honestly, this particular effect has never been confirmed in real people, so on its own it's pretty theoretical.
That said, verapamil is not a prodrug, so anything that slows its breakdown could let more of it build up. In theory that might mean lower blood pressure, dizziness, a slower heartbeat, swollen ankles, or constipation. Grapefruit is also known to affect a different enzyme (CYP3A4) that handles verapamil more directly, which is the bigger real-world concern. To be safe, check with your pharmacist or doctor before making grapefruit a regular habit.

While most in vitro research shows that grapefruit products inhibit P-gp,, research in humans is less clear. Two small clinical studies in healthy adults using digoxin as a probe substrate show that grapefruit juice does not inhibit P-gp in the kidneys. It is unclear whether this applies to intestinal P-gp, for which digoxin is not considered to be a sensitive probe. Grapefruit juice has been shown to reduce levels of fexofenadine, and increase levels of quinidine. However, as both of these drugs are also substrates of other enzymes and transporters, it is unclear what role, if any, intestinal P-gp has in these findings.

Here's the honest picture with grapefruit and verapamil. The interaction on file is about a transporter called P-glycoprotein, and frankly the human evidence is weak and conflicting. Grapefruit doesn't seem to block this transporter in the kidneys, and whether it does much in the gut is genuinely unclear.
That said, verapamil is also handled by an enzyme (CYP3A4) that grapefruit is known to slow down, so larger or regular amounts of grapefruit could let more verapamil build up. If that happens, you might notice more of its typical effects: dizziness, a slower heartbeat, constipation, or low blood pressure. An occasional segment is unlikely to cause trouble, but before making grapefruit a daily habit, check with your pharmacist or doctor.

About Verapamil
What Is Verapamil Used For?
Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil
Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.
Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
