Why this applies to Pentamidine

Pentamidine falls under QT INTERVAL-PROLONGING DRUGS — the group Grapefruit interacts with.

Grapefruit or grapefruit juice, especially if consumed in large amounts, can cause additive QT interval prolongation when taken with QT interval-prolonging drugs, potentially increasing the risk of ventricular arrhythmias.
Clinical research in healthy volunteers shows that drinking 6 liters of grapefruit juice over 6 hours prolonged the QTc by a peak amount of 14 milliseconds (ms). This prolongation was similar to the QT prolongation caused by the drug moxifloxacin. In individuals with long QT syndrome, a smaller dose of grapefruit juice, 1.5 liters, resulted in a greater peak QTc prolongation of about 30 ms. The effect of smaller quantities of grapefruit juice on the QT interval is unclear.

Here's something most people don't expect about grapefruit. Beyond the well-known enzyme effects, grapefruit juice in large amounts can slightly stretch out the heart's electrical cycle, what we call the QT interval. Pentamidine already does this on its own, so stacking the two together can add up and raise the chance of a dangerous irregular heartbeat.

The research behind this used very big amounts of juice (think liters at a time), so an occasional half of a grapefruit at breakfast is far less concerning. Still, since pentamidine is given for a serious infection and carries its own heart-rhythm risk, I'd play it safe. Talk with your doctor or pharmacist before drinking grapefruit juice regularly while you're on this medicine.

Pentamidine also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the short version: grapefruit can mildly block an enzyme called CYP2C19, and our records group pentamidine with drugs that this enzyme helps clear. If that pathway slows down, a bit more pentamidine could linger in your body, which in theory might nudge up the chance of side effects like low blood pressure, low blood sugar, kidney strain, or heart rhythm changes.

That said, this is mostly theoretical and based on lab and limited human data, not on pentamidine itself. Pentamidine is also given as an injection in a hospital or clinic, so casual grapefruit eating is unlikely to be a big deal. Still, it's worth a quick mention to your doctor or pharmacist, especially if you eat grapefruit regularly.
