Pentamidine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with this pairing. Pentamidine is one of those medications that can stretch out your heart's electrical rhythm, something we call QT prolongation. On its own, that can occasionally trigger a dangerous, fast heartbeat. Ephedra is a powerful stimulant that also stresses the heart and has been shown to cause EKG changes, even at normal doses.

Stacked together, the worry is that they pile on each other and raise the risk of a serious arrhythmia. Ephedra isn't a prodrug issue here; it simply adds its own heart strain on top of pentamidine's. Honestly, ephedra is banned in U.S. supplements for safety reasons, so I'd avoid it entirely. Please talk with your doctor or pharmacist about safer choices.

Pentam 300 (pentamidine isethionate for injection) is indicated for the treatment of pneumonia caused by Pneumocystis carinii (PCP), a potentially life-threatening lung infection that most commonly affects people with weakened immune systems.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
