Imipramine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with combining ephedra and imipramine. Imipramine is a tricyclic antidepressant, and one of its known effects is that it can stretch out the heart's electrical cycle (what we call QT prolongation). Ephedra's stimulants, ephedrine and pseudoephedrine, can do the same thing, even at the recommended doses studied in healthy people. Stacking the two together may add up and raise the risk of a dangerous heart rhythm.

On top of that, both speed up the heart and can push blood pressure higher, so you could feel a racing heartbeat, palpitations, or jitteriness. Ephedra is actually banned in U.S. supplements for good reason. Please don't change your imipramine, but do talk with your pharmacist or doctor and steer clear of ephedra products.

Imipramine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the bigger picture first: ephedra is a banned, risky stimulant, so I wouldn't pair it with imipramine for safety reasons alone. Both can raise heart rate and blood pressure, and stacking those effects is the real worry I'd have for you.

On the specific interaction we have on file, it's pretty mild and theoretical. Some animal studies hint that ephedra may slightly speed up a liver enzyme (CYP1A2) that helps break down imipramine. If that happened in people, your body might clear the drug a bit faster, which could make your depression harder to keep in check. We don't have solid human proof of this, though. Please loop in your pharmacist or doctor before combining anything with imipramine.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
