Why this applies to Asenapine

Asenapine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the concern with pairing these two. Asenapine can stretch out the heart's electrical reset time, something we call QT prolongation. Ephedra is a strong stimulant that can do the same thing, and it has caused EKG changes and a faster, irregular heartbeat even in healthy people taking normal doses. Put them together and those effects can stack up, which raises the risk of a dangerous heart rhythm problem.

Ephedra isn't a prodrug issue here; this is simply two things pushing the heart in the same risky direction. Honestly, ephedra is banned from supplements in the U.S. for good reason, so I wouldn't use it at all. Please talk with your pharmacist or doctor about safer choices before combining anything with your asenapine.

Asenapine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the basic idea: your body uses a liver enzyme called CYP1A2 to help break down asenapine (Saphris, Secuado). Some early animal and lab research hints that ephedra may speed that enzyme up. If it does, your body could clear asenapine a bit faster, which might leave less of it working and could make it less effective at controlling symptoms of schizophrenia or bipolar mania.

I want to be honest about how shaky this is. The evidence here is only test-tube and animal data, so it's theoretical, not proven in people. The much bigger concern is ephedra itself, which is banned in U.S. supplements because of dangerous effects on the heart and blood pressure. Please don't change your asenapine, and talk with your pharmacist or doctor before using anything containing ephedra.

What Is Asenapine Used For?

Brand names: Saphris, Secuado

Asenapine treats schizophrenia in adults and bipolar I disorder (manic or mixed episodes) in adults and in children ages 10 to 17. It can be used alone or alongside lithium or valproate in adults with bipolar I disorder.

The sublingual tablet forms (Saphris and the generic asenapine sublingual tablet) are approved for both schizophrenia and bipolar I disorder. Secuado, the skin patch, is approved only for schizophrenia in adults.
