Fluoxetine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing ephedra and fluoxetine (Prozac). Both can nudge the heart's electrical timing in the same direction, lengthening what's called the QT interval. Fluoxetine can do this on its own, and ephedra's stimulant compounds (ephedrine and pseudoephedrine) have caused EKG changes even in healthy people at normal doses. Stacking the two could add up and, in rare cases, trigger a dangerous irregular heartbeat.

On top of that, ephedra revs up your heart rate and blood pressure, which doesn't mix well with an antidepressant. Honestly, ephedra is banned in U.S. supplements for good reason, so I wouldn't use it at all. If you're taking fluoxetine and considering anything with ephedra, please check with your pharmacist or doctor about safer options first.

Fluoxetine is approved to treat four conditions: major depressive disorder (MDD), obsessive-compulsive disorder (OCD), bulimia nervosa (a binge-eating and purging disorder), and panic disorder — with or without agoraphobia (fear of situations that feel hard to escape).

It works for adults across all four conditions, and for children and teens with depression (ages 8–18) and OCD (ages 7–17).
