Here's the concern with combining these two. Nitrofurantoin, while it doesn't happen often, can occasionally stress the liver. Ephedra has its own track record of liver injury, with a number of reported cases tied to ephedra-containing products. The worry is that the two together could be harder on your liver than either one alone. I'll be honest though, this is a theoretical interaction based on how each acts in the body, not something proven to happen when they're used together.

Watch for signs your liver isn't happy: yellowing of the skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. Honestly, my bigger concern is ephedra itself, which is banned in U.S. supplements for good reason. Please loop in your pharmacist or doctor before mixing these.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
