Ephedra and Allopurinol

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Allopurinol

Allopurinol falls under HEPATOTOXIC DRUGS — the group Ephedra interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
There are numerous cases of liver toxicity from ephedra-containing supplements.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern when you pair these two. Ephedra has been tied to many reports of liver injury, and allopurinol, while usually well tolerated, can also occasionally stress the liver. Put them together and the worry is simply that the two strains on your liver could add up. This isn't about ephedra changing how allopurinol works for your gout or uric acid. It's about both being potentially hard on the same organ.

To be honest, this is a theoretical caution based on how these substances behave, not proven cases of the combo. That said, ephedra is banned in U.S. supplements for good reason, including those liver reports. Please loop in your pharmacist or doctor about safer options rather than using it at all.

From our pharmacist’s overview on the Allopurinol page

“Allopurinol (Zyloprim) prevents gout attacks by lowering uric acid, and I would give this list real weight, because 5 of the 60 supplements and herbs on file carry a major rating. The largest cluster, 38 ingredients, is products that can stress the liver on their own, and regular beer or wine sits in that major group, since alcohol on…”
