Here is the interaction we found between these two, with the evidence behind it.

Major

STIMULANT DRUGS

Why this applies to Pseudoephedrine

Pseudoephedrine falls under STIMULANT DRUGS — the group Ephedra interacts with.

Theoretically, concomitant use might increase the risk for serious adverse effects.
Drugs with CNS stimulant properties might increase the risk of hypertension and the adverse cardiovascular effects of ephedra.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing: pseudoephedrine (Sudafed) and ephedra are basically two stimulants pulling in the same direction. Pseudoephedrine isn't a prodrug, it's already active, and it works by tightening blood vessels and nudging your 'fight or flight' system. Ephedra does almost the exact same thing through ephedrine. So stacking them doesn't just add up, it can pile on.

Together you'd worry about a faster or pounding heartbeat, jitteriness, trouble sleeping, headache, and most importantly a real jump in blood pressure. The evidence here is more anecdotal than hard science, but the logic is solid and the risk is rated major, so I'd take it seriously. On top of that, ephedra is banned in U.S. supplements for good reason. Please check with your pharmacist or doctor before combining anything like this.

About Ephedra

What is Ephedra?

Ephedra
  • Weight loss
  • Nasal and sinus congestion
  • Asthma and breathing problems
  • Boosting energy and athletic performance
  • Allergies

Ephedra (ma huang) contains powerful stimulants like ephedrine that affect the heart and nervous system. Because it has been linked to serious harms including high blood pressure, stroke, and death, it is banned for use in dietary supplements in the U.S. and many other countries. We do not recommend using ephedra-containing supplements; talk to your pharmacist or doctor about safer, approved options.
