Here's the concern with pairing these two. Both ephedra and naltrexone can be tough on the liver. Naltrexone, at higher doses, has been known to bump up liver enzymes, and ephedra supplements have been tied to a number of reported cases of liver injury. Put them together and, in theory, that strain could add up. This isn't about ephedra changing how naltrexone works in your body, it's simply two things that may stress the same organ at once.

I'll be honest, this is a theoretical caution based on how these substances behave, not hard proof. Still, since ephedra is banned in U.S. supplements and carries real heart and stroke risks of its own, I'd steer clear of it entirely. Please loop in your doctor or pharmacist before combining anything with naltrexone.

What Is Naltrexone Used For?

Brand names: Trexan, Depade, Vivitrol

Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people with opioid dependence. The extended-release injectable suspension — sold as Vivitrol and also available as a generic injectable — is used to treat alcohol dependence and to prevent relapse to opioid dependence after a person has completed opioid detoxification.

All formulations are meant to be part of a broader treatment plan that includes counseling and other support — naltrexone works best when it is not used alone.
