Indapamide falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing ephedra and indapamide. Indapamide is a water pill that, in some people, can lower potassium and magnesium levels. When those minerals dip, your heart's electrical rhythm can stretch out a bit (what we call QT prolongation), which raises the risk of a dangerous irregular heartbeat. Ephedra is a stimulant that, even at usual doses, has been shown to nudge the QT interval in the same direction and trigger extra heartbeats.

Put them together and you may stack two QT effects on top of each other, which could mean palpitations, dizziness, or fainting. This is partly theoretical, but the potential outcome is serious. On top of that, ephedra can raise blood pressure and work against what indapamide is trying to do. Ephedra is banned in U.S. supplements anyway. Please talk with your pharmacist or doctor about safer options.

What Is Indapamide Used For?

Brand names: Lozol, Lozide

Indapamide tablets are used to treat high blood pressure (hypertension), either on their own or together with other blood pressure medications. Lowering blood pressure helps reduce the risk of related complications.

Indapamide tablets are also used to treat salt and fluid retention (edema) associated with congestive heart failure. In certain cases, indapamide may also be appropriate during pregnancy when swelling is caused by a medical condition rather than normal pregnancy changes — but routine use in otherwise healthy pregnant women is not recommended.
