Bedaquiline falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing these two. Bedaquiline is well known for stretching out part of your heart's electrical cycle, something we call the QT interval. Ephedra is a stimulant, and even at normal doses it has been shown to nudge the heart rhythm and lengthen the QT interval too. Put them together and those effects can stack, which raises the risk of a dangerous irregular heartbeat (an arrhythmia).

I'd treat this as a real concern, not just a theoretical one. On top of that, ephedra is banned in U.S. supplements because of strokes and heart problems on its own. The safest move is to avoid ephedra entirely and ask your doctor or pharmacist about safer options while you're being treated for TB.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
