Bedaquiline falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with these two together. Bedaquiline already carries a warning that it can stretch out the heart's electrical cycle (called the QT interval). American Hellebore contains potent alkaloids that may do the same thing. When two things both lengthen the QT interval, the effects can add up, which raises the risk of a dangerous heart rhythm called torsades de pointes (a rapid, chaotic heartbeat that can cause fainting or worse).

On top of that, this plant is genuinely poisonous and can drop your heart rate and blood pressure sharply, even in tiny amounts. The evidence here is moderate, but given how toxic the plant is, I'd steer clear. Please don't take anything containing American Hellebore while on your TB treatment without first talking to your doctor or pharmacist.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
