Sotalol falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with putting these two together. Sotalol already works by lengthening the heart's electrical reset time, what doctors call the QT interval. That's actually part of how it controls irregular heartbeats, but it's also why it carries a real risk of a dangerous rhythm called torsades de pointes. American Hellebore (Veratrum viride) contains toxic alkaloids that can stretch out that same QT interval and, on their own, slow the heart and drop blood pressure to dangerous lows.

Stack those effects and you could push the heart into fainting, palpitations, or a life-threatening rhythm. Honestly, American Hellebore isn't a safe supplement to begin with, it can poison you even in small doses. Please don't combine it with sotalol, and talk with your doctor or pharmacist first.

About Sotalol

What Is Sotalol Used For?

Brand names: Betapace AF, Betapace, Sotacor, Sorine

Sotalol is used to treat documented, life-threatening ventricular arrhythmias (dangerous irregular heart rhythms originating in the lower chambers of the heart), such as sustained ventricular tachycardia (a rapid, sustained abnormal heartbeat). Because it carries real risks of its own, it is generally reserved for arrhythmias serious enough to be life-threatening — not for mild or symptom-only rhythm problems.

It is not recommended for people with less severe arrhythmias, even if those arrhythmias cause symptoms, and it should be avoided in people with asymptomatic (no-symptom) extra heartbeats called ventricular premature contractions.
