Let me start with the bigger picture: American Hellebore is genuinely poisonous, and I'd steer clear of it entirely unless a doctor is supervising. Beyond its toxicity, here's the concern with your inhaler. Indacaterol (Arcapta) is a long-acting bronchodilator that can already nudge your heart's electrical timing, something called the QT interval. American Hellebore's alkaloids can do the same thing.

Stack the two together and you raise the odds of a dangerous, irregular heart rhythm (torsades de pointes), which can feel like a racing heartbeat, dizziness, or fainting. The evidence here is on the lighter side and the risk is possible rather than certain, but the stakes are high. Please run this by your pharmacist or doctor before mixing them.
