American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with this pairing. Quinidine already affects your heart's electrical timing, stretching out what we call the QT interval. That's actually part of how it works, but it also means quinidine carries a known risk of a dangerous rhythm called torsades de pointes. American hellebore appears to push the QT interval in the same direction, so putting the two together could stack that risk and lead to fainting, a racing or chaotic heartbeat, or worse.

On top of that, hellebore itself is genuinely toxic and can crash your heart rate and blood pressure. The QT evidence here is modest, but with a heart rhythm drug like quinidine, even small added risk matters. Please don't combine these without talking to your doctor or pharmacist first.

It is not a safe self-care supplement, and any non-homeopathic product carries a serious risk of poisoning. About Quinidine

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
