Clarithromycin falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with putting these two together. Clarithromycin can already stretch out the heart's electrical reset time (what we call the QT interval). American hellebore can do the same thing on its own. When you stack two things that both push in that direction, the risk of a dangerous, fast heart rhythm called torsades de pointes goes up. Warning signs would be a racing or pounding heartbeat, dizziness, fainting, or feeling like you might pass out.

This isn't about clarithromycin being broken down differently. It's two separate effects on the heart adding up. The evidence here is on the lighter side, but the potential outcome is serious, and honestly American hellebore is a poisonous plant I wouldn't recommend using at all. Please talk to your pharmacist or doctor before combining these.

American Hellebore (Veratrum viride) is a highly poisonous plant whose root contains toxic alkaloids that can cause dangerous drops in heart rate and blood pressure, even in small amounts. It is not a safe self-care supplement, and any non-homeopathic product carries a serious risk of poisoning. Do not use it without medical supervision, and talk to a pharmacist or doctor before considering any product that contains it.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
