Doxepin falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with pairing these two. Doxepin, like other tricyclic antidepressants, can stretch out the heart's electrical recovery time (what doctors call the QT interval) on an ECG. American Hellebore contains toxic alkaloids that may do the same thing on their own. Stack the two together and you raise the odds of a dangerous, irregular heartbeat called torsades de pointes, which can cause fainting or worse.

This isn't about Doxepin building up in your body. It's two separate things tugging on your heart rhythm at the same time. On top of that, American Hellebore is genuinely poisonous and not a safe self-care product. Please don't combine these without first talking to your pharmacist or doctor.
