Fluoxetine falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the heart of it: fluoxetine (Prozac) can, on its own, gently stretch out a part of your heartbeat called the QT interval. American Hellebore can do the same thing, plus it's genuinely poisonous and can drop your heart rate and blood pressure to dangerous levels. Put the two together and you stack those effects, which raises the risk of a rare but serious rhythm problem called torsades de pointes.

This isn't a prodrug situation, so the concern is simply additive. The evidence here is on the weaker side (mostly theoretical for the hellebore part), but because the plant itself is so toxic, I'd steer clear entirely. Please talk with your pharmacist or doctor before going anywhere near a product containing it.
