Azithromycin falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

Here's the heart of it: azithromycin can stretch out the heart's electrical reset time, something doctors call QT prolongation. American hellebore can do the same thing through its toxic alkaloids. Put the two together and you stack one heart-rhythm risk on top of another, which raises the chance of a dangerous irregular beat called torsades de pointes.
This isn't about how the drug is broken down, so it doesn't matter that azithromycin works on its own (it's not a prodrug). The concern is simply two QT-prolonging things at once. Watch for fainting, a racing or pounding heartbeat, or feeling lightheaded. American hellebore is genuinely poisonous and not a safe self-care product, so please talk with your pharmacist or doctor before going anywhere near it.

About Azithromycin
What Is Azithromycin Used For?
Brand names: Zithromax, Zmax, Azasite
Azithromycin treats mild to moderate infections caused by susceptible bacteria. Oral tablets and oral suspension are used for several infection types in adults and, for some, children.
Oral uses include acute bacterial flare-ups of chronic bronchitis, acute bacterial sinusitis, uncomplicated skin infections, urethritis and cervicitis, genital ulcer disease in men, ear infections (acute otitis media) in children, community-acquired pneumonia, and pharyngitis/tonsillitis. The injection (Zithromax and azithromycin for injection) is used for community-acquired pneumonia in adults and pelvic inflammatory disease. AzaSite eye drops are used only for bacterial conjunctivitis (pink eye).
