Procainamide falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with pairing these two. Procainamide already affects your heart's electrical timing, and it can stretch out what's called the QT interval. American Hellebore can do the same kind of thing through its toxic alkaloids. When you stack two QT-stretching effects on top of each other, you raise the risk of a rare but dangerous rhythm called torsades de pointes, which can cause fainting, a racing or chaotic heartbeat, or worse.

On top of that, this plant can sharply drop your heart rate and blood pressure on its own, so layering it onto a heart-rhythm drug is risky. The evidence here is limited, but procainamide is a sensitive medication and Hellebore is genuinely poisonous. Please don't combine them, and talk with your doctor or pharmacist first.

About Procainamide

What Is Procainamide Used For?

Brand names: Pronestyl, Procanbid, Procan SR, Procan, Pronestyl-SR

Procainamide hydrochloride injection is used to treat documented, life-threatening ventricular arrhythmias — most notably sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers of the heart). Because of its serious risk profile, it is reserved for situations where the benefit clearly outweighs the risk.

Procainamide is not recommended for minor or asymptomatic arrhythmias, and treatment should always be started in a hospital setting where close monitoring is possible. Antiarrhythmic drugs, including procainamide, have not been shown to improve survival in patients with arrhythmias that are not life-threatening.
