Amitriptyline falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing ephedra and amitriptyline (Elavil). Amitriptyline can stretch out part of your heart's electrical cycle, called the QT interval. Ephedra, which is a strong stimulant, can do the same thing and also speed up your heart and raise your blood pressure. Put them together and those effects can stack up, which raises the risk of a dangerous heart rhythm.

On top of that, amitriptyline already tends to cause a racing or pounding heartbeat in some people, so adding a stimulant isn't a good mix. Ephedra is banned in U.S. supplements for safety reasons anyway. Please don't combine these without talking to your pharmacist or doctor first, and ask about safer options.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the situation with ephedra and amitriptyline (Elavil). Your body uses a liver enzyme called CYP1A2 to help break down amitriptyline. Some animal studies suggest ephedra might speed that enzyme up, which could clear the medication out a little faster and possibly make it work less well for your mood. I want to be honest though: this is only theoretical and based on lab and animal data, so it may not happen much in people.

The bigger concern in real life is that ephedra is a strong stimulant, and amitriptyline can also affect your heart rhythm and blood pressure. Stacking the two could raise your heart rate or blood pressure. Ephedra is banned in U.S. supplements anyway, so please talk with your pharmacist or doctor about safer options before using it.
