Here's the concern with pairing ephedra and rosiglitazone (Avandia). Both have been tied to liver stress on their own. Rosiglitazone, like other diabetes pills in its class, can occasionally affect the liver, and ephedra-containing products have a track record of liver toxicity too. The worry is that using them together could add up and put extra strain on your liver. Warning signs would include yellowing of the skin or eyes, dark urine, belly pain, nausea, or unusual tiredness.

I'll be honest: this is a theoretical interaction based on how these products behave, not proven cases of the two combined. Even so, ephedra is banned in U.S. supplements for good reason. Please talk with your pharmacist or doctor before using any ephedra product, and ask about safer options.

Theoretically, taking ephedra with antidiabetes drugs might interfere with blood glucose control.
One study in animals shows that some components of ephedra may lower blood glucose levels. However, most human research suggests that ephedra and ephedrine, a component of ephedra, can raise blood glucose levels and might decrease the effectiveness of drug therapy. Monitor blood glucose concentrations closely.

Here's the short version: rosiglitazone (Avandia) works to lower your blood sugar, while the stimulants in ephedra tend to push your blood sugar back up. Ephedrine revs up your 'fight or flight' system, and that stress response can raise glucose levels. So the two can work against each other, and your Avandia may not control your sugar as well as it should.

This is mostly a theoretical concern based on lower quality studies, but the direction is pretty consistent in people. On top of that, ephedra itself is banned in U.S. supplements because it can dangerously raise blood pressure and strain the heart. Please don't use it. If you're looking for help with energy, weight, or blood sugar, talk with your pharmacist or doctor about safer, approved options.

What Is Rosiglitazone Used For?

  • Rosiglitazone is used along with a diet and exercise program and sometimes with one or more other medications to treat type 2 diabetes (condition in which the body does not use insulin normally and therefore cannot control the amount of sugar in the blood). Rosiglitazone is in a class of medications called thiazolidinediones. It works by increasing the body's sensitivity to insulin, a natural substance that helps control blood sugar levels. Rosiglitazone is not used to treat type 1 diabetes (condition in which the body does not produce insulin and therefore cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
