Safinamide Mesylate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Ephedra interacts with.

Theoretically, concomitant use might increase the risk of serious adverse effects.
Concomitant use of ephedra with MAOIs might increase the risk of hypertension or hypotension, hyperpyrexia, hallucinations, convulsions, rhabdomyolysis, and urinary problems.

Here's the concern with this pairing. Safinamide (Xadago) is an MAO-B inhibitor, and ephedra's stimulants (ephedrine and pseudoephedrine) push your body's fight-or-flight system into high gear. When you slow down how those stimulants are broken down and pile on a strong stimulant at the same time, the effects can stack up in a dangerous way. We worry about a sharp spike (or swing) in blood pressure, a racing heart, dangerously high body temperature, confusion or hallucinations, and even seizures.

Honestly, ephedra is banned in U.S. supplements for good reason, so I wouldn't use it regardless. But combined with safinamide, the risk is real, not just theoretical. Please don't start or stop anything on your own. Talk with your pharmacist or doctor about safer options first.
