Grapefruit and Aspirin, Pentazocine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Aspirin, Pentazocine

Aspirin, Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP1A2.
In vitro research suggests that grapefruit juice might inhibit CYP1A2 enzymes. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Talwin Compound combines aspirin with pentazocine, a prescription pain reliever. The concern here is that grapefruit can block an enzyme called CYP1A2, which helps your body process pentazocine. If that enzyme is slowed, a little more of the drug could hang around, in theory making effects like drowsiness, dizziness, nausea, or confusion a bit stronger.

Here is the reassuring part: this interaction has only shown up in lab (test-tube) studies, not in actual people. So the real-world risk looks low, and an occasional grapefruit probably is not a big deal. Still, since both ingredients can affect how alert you feel, it is worth a quick chat with your pharmacist or doctor before making grapefruit a daily habit.

From our pharmacist’s overview on the Aspirin, Pentazocine page

“Talwin Compound pairs pentazocine, an opioid painkiller, with aspirin, and I read its supplement list with more concern than most. The great majority of entries do one of two things: raise the bleeding risk that aspirin already carries, or deepen the sedation pentazocine causes.”
