Why this applies to Atovaquone, Proguanil
Atovaquone, Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Grapefruit interacts with.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the nuance with Malarone (atovaquone and proguanil) and grapefruit. The proguanil part is actually a prodrug, meaning your body uses an enzyme called CYP2C19 to convert it into its active, malaria-fighting form (cycloguanil). Grapefruit is thought to slow that same enzyme down a bit. In theory, that could mean less of the active compound forms, which is the opposite of what you want when you're relying on this medicine to prevent or treat malaria.
That said, this concern is mostly theoretical and based on lab work and a study using a different drug, so it isn't well proven for Malarone. Still, given that this is malaria protection where effectiveness really matters, I'd run it by your pharmacist or doctor before making grapefruit a regular habit while you're on it.

What Is Atovaquone and Proguanil Used For?
Brand name: Malarone
Atovaquone and proguanil is used for two purposes: preventing Plasmodium falciparum (P. falciparum) malaria before and during travel to areas where it is present, and treating acute, uncomplicated P. falciparum malaria once infection has occurred. This includes areas where other antimalarials such as chloroquine, mefloquine, halofantrine, or amodiaquine may have unacceptable failure rates due to drug resistance.
These uses apply across the supported products: Atovaquone and Proguanil Hydrochloride tablets, Atovaquone and Proguanil Hydrochloride Pediatric tablets, and Malarone tablets. Pediatric formulations are dosed by body weight for children; adult-strength formulations are used for adults and heavier children.
