Atovaquone, Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the connection with Malarone (atovaquone and proguanil). The proguanil part is actually a prodrug, meaning your body uses an enzyme called CYP2C19 to convert it into its active form, cycloguanil, which helps fight the malaria parasite. In lab studies, bacopa appears to mildly block that same CYP2C19 enzyme.

If that blocking happened in real life, it could mean a bit less of the proguanil gets activated, which in theory might make it slightly less effective at preventing or treating malaria. I want to stress this is purely theoretical, based only on test-tube research, and we don't know if it matters in actual people. Still, malaria is serious, so I'd skip the bacopa and let your doctor or pharmacist know what you're taking before you travel.

Bacopa is an Ayurvedic herb most often used for memory and thinking. What Is Atovaquone and Proguanil Used For?

Brand name: Malarone

Atovaquone and proguanil is used for two purposes: preventing Plasmodium falciparum (P. falciparum) malaria before and during travel to areas where it is present, and treating acute, uncomplicated P. falciparum malaria once infection has occurred. This includes areas where other antimalarials such as chloroquine, mefloquine, halofantrine, or amodiaquine may have unacceptable failure rates due to drug resistance.

These uses apply across the supported products: Atovaquone and Proguanil Hydrochloride tablets, Atovaquone and Proguanil Hydrochloride Pediatric tablets, and Malarone tablets. Pediatric formulations are dosed by body weight for children; adult-strength formulations are used for adults and heavier children.
