Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the gist: bacopa might slow down a liver enzyme called CYP2C19, and on paper that could let drugs cleared by that enzyme build up a bit. Formoterol, your long-acting inhaler for COPD, isn't a prodrug, so if levels nudged upward you'd expect a touch more of its usual effects, things like a faster or pounding heartbeat, shaky hands, a jittery feeling, or a headache.

The important part: this concern comes only from lab (test tube) studies, not from people, so we honestly don't know if it matters in real life. Formoterol is also mostly broken down by other pathways, which makes a big problem unlikely. Still, if you notice more jitters or a racing heart, mention it to your pharmacist or doctor before changing anything.

Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the short version on combining bacopa with your formoterol inhaler. The concern on file is that bacopa might slow down a liver enzyme called CYP2C9, which could let certain drugs build up. In theory, higher formoterol levels could mean more of its typical side effects, like a fast or pounding heartbeat, shaky hands, restlessness, or a headache.

But I want to keep this in perspective. This is based only on lab studies, and even those disagree, so it's far from proven in real people. Formoterol is also inhaled, so very little reaches the liver to begin with. The interaction is theoretical and likely small. Still, if you notice jitteriness or a racing heart, mention it to your pharmacist or doctor.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
