Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist with milk thistle and your formoterol inhaler. Formoterol is partly handled by a liver enzyme called CYP2C9, and there's a theory that milk thistle's active part (silymarin) might slow that enzyme down a little. If it did, your body could clear formoterol slightly slower, which in theory might nudge up the usual beta-agonist side effects like a faster or pounding heartbeat, shakiness or tremor, headache, or feeling jittery.

The good news: the evidence here is shaky and conflicting, and this is rated as a minor and unlikely interaction. Most people won't notice anything. Since your inhaler delivers a low dose to your lungs, the effect is likely to be small. Still, if you start milk thistle and feel extra jittery or notice your heart racing, mention it to your pharmacist or doctor.

About Formoterol Oral Inhalation

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
