Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the deal with this combo. Your body clears cabotegravir mainly through a process called glucuronidation, which uses an enzyme group known as UGT. In lab studies, milk thistle's silymarin compounds appear to slow down those same UGT enzymes. In theory, that could mean cabotegravir clears a little more slowly and its levels creep up, which might raise the chance of side effects like headache, fever, fatigue, or injection-site soreness.

I want to be honest, though: this is based only on test-tube and animal research, so we really don't know if it matters in real people. Since cabotegravir is so important for HIV treatment or prevention, I wouldn't just wing it. Check with your pharmacist or doctor before adding milk thistle so they can keep an eye on you.

About Cabotegravir

What Is Cabotegravir Used For?

Brand name: Apretude

Vocabria (cabotegravir) tablets are used in combination with rilpivirine (Edurant) to treat HIV-1 infection in adults and adolescents aged 12 and older who weigh at least 35 kg and whose HIV is already virologically suppressed (virus levels undetectable or very low on their current regimen). Vocabria tablets serve as a short-term oral lead-in — taken for roughly one month — before switching to long-acting injectable therapy.

Cabotegravir tablets are also used as an oral lead-in for HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative adults and adolescents (weighing at least 35 kg) confirm they tolerate the drug before starting injectable PrEP. Both uses require confirmed HIV-negative status before starting.
